Message to staff: Vaccination Notice

Posted on July 15, 2021

Dear UP colleagues

The current national vaccination programme for people 35 years of age and older is currently under way. A vaccination site has been established at the Faculty of Health Sciences’ Prinshof Campus.

If you receive this communication but have already received a vaccination at another vaccination station, please still complete the UP Questionnaire (Step 2), so that we don’t contact you unnecessarily.

All staff members who are eligible by age, should please ensure that they follow the required steps below in order  to receive a booking date and time via SMS, to receive their vaccination.

Step 1: Please click on the following link, to register yourself for a vaccination (currently only available to individuals 35 years or older): https://vaccine.enroll.health.gov.za/#/

Step 2:  Please complete the UP Google form*Google Form.  Once completed, the form will automatically add your details to the booking list, after which you will be enrolled to receive an SMS detailing your appointment.

If clicking on "Google form" does not work, please use this URL:

https://forms.gle/rBgY9CDTUyvAL4Up8

Step 3:  Wait for us to contact you regarding your appointment via SMS notification.

Should you have any queries or concerns, please do not hesitate to contact the team: email[email protected], or telephone on: 0607368019

PLEASE NOTE:

- We will not accept any walk-ins at the UP vaccination site.

- Only persons who have received an SMS appointment notification from us, and are therefore booked on the list to be vaccinated on a given day, will be allowed to enter the site.

Line managers who are aware of staff without access to email must please inform them of the process and assist where possible to enable their colleagues to make use of the opportunity.

Staff who have been vaccinated should continue to comply with all the COVID-19 protocols, as they can still contract and/or spread the virus. Please remember that practising the basic prevention protocol of wearing masks, adhering to social distancing and washing of hands remains the most effective way to guard against the spread of the COVID-19 virus.

I encourage those who are eligible to vaccinate to do so to minimise the risk of infection and illness.

Best regards,
