Dear UP colleagues

As you know, the current national vaccination programme for people 60 years of age and older is currently under way. A vaccine site has been established at the Faculty of Health Sciences’ Prinshof Campus.  

If you receive this communication but have already received a vaccination at another vaccination station, please still complete the UP Questionnaire (Step 2), so that we don’t contact you unnecessarily. 

For staff members who are eligible by age, please ensure that you follow the required steps below, to receive a booking date and time via SMS, for when you can receive your vaccination. 

Step 1: Please click on the following link, to register yourself for a vaccination (currently only available to individuals 60 years or older): https://vaccine.enroll.health.gov.za/#/

Step 2:  Please complete the UP Google form*: Google Form. Once completed, the form will add your details automatically to the booking list, after which you will be enrolled to receive an SMS appointment.

Step 3:  Wait for us to contact you for your appointment via SMS notification.

 PLEASE NOTE:

- We will not accept any walk-ins at the UP vaccination site.

- Only persons who have received an SMS appointment from us, and are therefore booked on the list to be vaccinated for a given day, will be allowed to enter the site.

 Line managers who are aware of staff without access to email must please inform and assist where possible to enable their colleagues to make use of the opportunity.

Please remember that practising the basic prevention protocol of wearing masks, adhering to social distancing and washing of hands remains the most effective way to guard against the spread of the COVID-19 virus.

Staff who have been vaccinated should continue to comply with all the COVID-19 protocols, as they can still contract and/or spread the virus. If you receive communication but have already received a vaccination at another station, please fill in the UP Questionnaire (Step 1), so that we don’t contact you unnecessarily.
