UP Emergency Booklet
  • The UP Emergency Booklet is a guide that provides instructions to UP community members on any of our campuses or residences about what to do in case of an emergency. 
Indemnity and Disclaimer form
  • A compulsory document to be completed before undertaking excursions or attending student events, or partaking in similar activities.

Accident/Incident/Near-miss preliminary report
  • Document incidents such as falls, spillages and fire within UP premises or for UP work outside of the institution. Please complete and forward to your line manager/ faculty manager/ res manager for investigation.
Employer's Report (staff only)
  • For injuries on duty (IODs), please complete and forward to your line manager. A copy of your ID and latest payslip are required.
