Instructions: Loading and Registering PPIP Version 3
When you open PPIP3 for the first time after installing it, it will ask you for a registration number. You have 30 days to register your PPIP3 by completing the form below to provide us with the following information:
(1) title, name and surname;
(2) facility/hospital name (max 40 characters);
(3) telephone number; and
(4) email address.
