Application for approval of amendments
A cover letter from the Principal Investigator must accompany the amended and supporting documents.
Please note that it is the duty of the investigator to clearly indicate in the cover letter how the amended document has been changed from the original and the reasons must be supplied that necessitated these changes. Also indicated whether any ethical implications are foreseen.
Any amendment submitted to the Faculty of Health Sciences, Research Ethics Committee, University of Pretoria, for approval must have a “footer” reflecting the version number and date that is applicable to the changes.
