Request for Access to the UP WWW Server

Please also very carefully read the Technical Specifications before you start coding your web pages.

The person in whose name this form is completed, will be responsible for coordinating web activities within their department/school/faculty and will take full responsibility for the correctness, content and maintenance of the relevant web pages, as well as for the safekeeping of the userid and password allocated for web access. Students or part time employees may not be appointed for this job, except in the case of a student organisation or hostel. The web and web editor need to be able to contact the responsible person immediately if any problems arise with the web pages. The information contained in this form is used to maintain a special database of web representatives. Therefore, if the web representative leaves the employ of the University of Pretoria, a new web representative must be registered.

The Department of Marketing Services reserves the right to remove any web page(s) not adhering to the policies and standards of the University of Pretoria. Notification thereof will be sent to the person responsible for the web page(s), as appointed by the head of the relevant department. The removal will be actioned thirty days after notification. The reason for the removal of the page(s) will be clearly stated.

The web editor, Department of Marketing Services, might also need to change web pages at short notice. Each department/school/faculty must authorize the web editor, in writing, to have the right to change web pages for the relevant department/school/faculty without prior notice.

In the case of crude language, racism, sexism or other forms of discrimination that might be harmful to the image of the University of Pretoria, or pornography appearing on web pages, those pages will be removed with immediate effect and the head of the relevant department/school/faculty notified thereof in writing.

Where a URL (Uniform Resource Locator) changes, the webmaster must be informed thirty days in advance in order to facilitate the changeover. If the web editor or webmaster changes a URL, the relevant faculty/school/department web representative will be advised thirty days in advance.

Please complete the form on the screen and then print and sign it. Please also complete only one form per department/school/faculty. Only one representative may be appointed at a time. The form has to be signed by both the Dean/Director/Head of Department (not the prospective representative's supervisor if not one of the aforementioned) and the prospective web representative.

Send the completed form to: Client Services, Department of Information Technology, University of Pretoria, Pretoria, 0002.

You will be requested telephonically to personally collect the userid and password. No userids or passwords will be given telephonically or sent via e-mail. This is in line with the rules and regulations as stipulated by the University's Security Policy.

A new form has to be completed in each of the following cases:

Unfortunately, it has happened in the past that unauthorised persons have tried to gain access to web pages by pretending that they are the web representative that has "forgotten" the password, or that web pages have been altered by somebody other than the web representative during the representative's absence, and that important information has been lost in such cases. A number of persons at both the Departments of Information Technology and Marketing Services also need to know who is responsible for which web pages should problems should arise. The information contained in this form is entered into a database that is accessed by the responsible staff. It is therefore imperative that the data in the database is up to date at all times.
